Re: Recommendation for Suspension —Unprofessional Conduct and Unsatisfactory Performance
On May 17th, 2015, we met to discuss your continued tardiness. On June 15th, you did not call until 9:05 am and did not arrive at work until 9:35 am. The following steps have been taken in the past to make you aware of the severity:
Verbal Warning: February 16, 2015
Written Warning: March 3, 2015
1-Day suspension: May 17, 2015
Excessive tardiness is unprofessional and is prohibited by the SCCOE Code of Conduct, which you have read and signed (attached). Your actions resulted in great disruption, as discussed in your prior written warning, for the Technology Resource Advisors/Applications Support team, resulting in a serious reduction of trust and credibility with our customers.
Directives
When not leading a training or at a customer meeting, you are expected to be at your workstation and ready to begin work at 9:00 am (you are also expected to arrive at trainings and meetings at the expected start time). If you are not able to be at your workstation by 9:00 am, you are to call me at 408-453-6726. If I am not able to answer, you are to call Lucy Lopez at 408-453-XXXX. In such circumstances, upon your arrival you are to immediately check in with me or, if I am not available, either Buck Rogers or Lucy Lopez.
In addition, you are to familiarize yourself with the SCCOE’s Living Our Values: Code of Conduct Handbook (attached), and especially review the section entitled, “Accountability and Reporting”, page 10.
As we have discussed on numerous occasions, most recently March 3, and April 17, your inability to report to work on time will not be tolerated and could lead to further disciplinary action up to and including termination.
A copy of this Notice will be placed in your personnel file ten (10) days after receipt. You are entitled to respond and to have your response attached to this notice if received by the Human Resources Office within those 10 days. Failure to comply with the above directives may lead to further disciplinary actions up to and including additional suspension without pay and termination.
